Abstract

DM is a chronic disease that affects a variety of organs and systems. Affecting also MGS causing diabetic patients to experience DED symptoms.
The pathogenesis of the ocular affection has many theories such as decreased corneal sensitivity, that lowers the blinking rate, leading to disruption of the lipid layer of the tear film and resulting in excessive evaporation, peripheral neuropathy also affects contraction of orbicularis ms and Riolan’s ms which in turn affects blinking that helps in applying contraction force on tarsal plates and MGs included and helping release of meibum. Some hypothesized that DM favors growth of bacteria and meibomitis inducing inflammation and hyper keratinization of the MGs causing eventually MGD.
AS-OCT is a non-invasive ocular imaging modality that gives detailed imaging of anterior segment structures and allows us to measure the tear film parameters with accuracy.
So, the goal of this study was to evaluate the impact of glycemic control on tear film and MGs function using AS-OCT and Sirius Pentacam meibogrphy to for evaluation of tear film parameters and MG dropout.
This was a case control study which included a total of 120 eyes of 120 participants (40 eyes of 40 normal controls, 40eyes of 40 diabetic patients with controlled HbA1c, and 40 eyes of 40 diabetic patients with uncontrolled HbA1c). The study was performed at Minia University Hospital and (IEC).
All patients had a complete ophthalmological evaluation and history taking before performing TBUT, Schirmer 1 test AS-Oct and meibography. AS-OCT parameters included TMH, TMA and TMD. Meibography parameters included percentage of MGs loss in both UL and LL.
In group A (normal controls), the age ranged between 30-50 years old. TMH median: 271 (216.25-367), TMA median 0.03 (0.02-0.04), TMD median 204.50 (167-249.50).
Percentage of MGs loss in UL 24.45 (15.25-37), Percentage of MGs loss in LL 21.85 (18.50-39.97).
In group B (diabetic patients with controlled HbA1c), the age ranged between 30-50 years old.
TMH median: 306 (235.25-388.75), TMA median 0.03 (0.02-0.05), TMD median 206.50 (161.75-251).
Percentage of MGs loss in UL 42.8 (31.43-49.6), Percentage of MGs loss in LL 31.8 (24.7-35.4).
In group C (diabetic patients with uncontrolled HbA1c) the age ranged between 30- 50 years old.
TMH median: 266.50(200-328.25), TMA median 0.02 (0.01-0.03), TMD median 169.50 (147.50-203.25).
Percentage of MGs loss in UL 43.1 (35.2-54.88), Percentage of MGs loss in LL 41.35 (30.85-58.03).
