Abstract

The aim of the present study was to evaluate Gestational sac
diameter and shape, yolk sac size and shape, embryonic heart rate and
Crown rump length during the first trimester as prognostic factors of
first trimester pregnancy outcomes.
The present study was prospective observational cohort study
carried out on 118 patients in Shebin Elkom teaching Hospital and
Menoufia University Hospital, outpatient clinic and obstetrics and
gynecology department in uncomplicated singleton pregnancy from
(20-35) years old from October 2022 to October 2023.
Pregnant women were divided into four groups group (1)
included women who were examined during (6-7weeks +6days).
group (2) included women who were examined during (8-
9weeks +6days).
group (3) included women who were examined during (10-
12weeks).
group (4) included women whose their pregnancies resulted
into first trimester spontaneous miscarriage.
All pregnant women attended antenatal clinic in their first
trimester of pregnancy were counseled recruited for the study.
Ultrasound was done for all women for measurement of gestational
sac and yolk sac diameter, CRL and fetal heart rate. All pregnancies
were followed for their pregnancy outcome after completion of 12
weeks by a subsequent ultrasound scan.

The present study showed that:
● There was a highly statistically significant decrease in gestational
sac diameter, fetal heart rate and CLR diameter in fetal loss
group more than ongoing pregnancy at 6, 9 and 12weeks.
● Cut off value of gestational sac diameter at 6, 9 and 12 weeks to
predict the outcome of pregnancy. It was 15mm at 6 weeks. At 9
weeks, the cutoff value was 27mm. At 12 weeks, the cutoff value
was 57.5mm.
● Cut off value of fetal heart rate at 6, 9 and 12 weeks to predict
the outcome of pregnancy. It was 104 at 6 week. At 9 weeks, the
cutoff value was 126. At 12 weeks, the cutoff value was 117.
● Cut off value of CLR at 6, 9 and 12 weeks to predict the outcome
of pregnancy. It was 8.5mm at 6 weeks. At 9 weeks, the cutoff
value was 14.5mm. At 12 weeks, the cutoff value was 33mm.
● there was a highly statistically significant increase in yolk sac
diameter in fetal loss group more than ongoing pregnancy at 6,9
and 12weeks.
● Cut off value of yolk sac diameter at 6, 9 and 12 weeks to predict
the outcome of pregnancy. It was 3.75mm at 6 weeks. At 9
weeks, the cutoff value was 5.25mm. At 12 weeks, the cutoff
value was 4.75mm.
