Abstract

This study was conducted on 20 patients with a clinical diagnosis of fistula in ano and 10 patients with a clinical diagnosis of perianal suppuration. There were 26 males and four females, thier ages rranged from 20 to 55 years with a mean 28.4. All patients were sujected to digital examination and anal endosonography. The results were compared with the operative findings.
Anal endosonography succeeded to identify 90% of the perianal abscesses. It also could demonstrate 75 % of fistulous tracks with respect to their location and relation to the anal sphincter muscles and 73.7 % of the internal openings.
Anal endosonography is a new, potentially promising technique for localising fistula tracks and associated abscesses in the perianal region. The problem with the currently available 7 MHz probe is that anatomical definition is poor beyond the internal sphincter. Hwoever, internal openings of fistulas can be accurately localised and a concomitant intersphincteric abscess may be identffied.
Image definition in the region beyond the internal anal sphincter, the isehiorectal fossa and the pelvic floor is expected to become more clear with the development of higher resolution scanners
We concluded that anal endosonography is a good and non-invasive method for imaging the anal sphincter complex that is useful in the preoperative assessment of patients with perianal suppurations and fistula-inano. It is recommended to be as a routine clinical procedure for evaluation of the anal anatomy and morphology.
