Abstract Acute rupture of the lateral ligament . of the ankle is a very important subject as it is commonly met with in practice and frequently missed by . the unexperienced physicians. Imperfectly diagnosed and inadequately treated tom lateral ligament cauld lead to serious disabilities. Controversy exists, not only for the best method provided an accurate diagnosis but also for the optimal treatment of injuries of the lateral ankle ligament. Many recommonded no operative treatment for all injuries and if ankle remains unstable, reconstructive surgery can be offored with confidence of obtaining patient satisfication. The aim of this work is to evaluate the value of immediate surgical repair of sever lateral ligament injuries of the ankle. We have reviewed the literature with regard to anatomy, biomechanics, pathology, diagnosis and treatment of the lateral ligament injuries of the ankle. The material of this study consisted of twenty five cases with mean age of 29.3 years presented with sever inversion injury of the lateral ankle ligament. All cases have been subjected to a carfull clinical examination for swelling, ecchymosis & point of maximum tenderness and stress testing for ankle instability in anteroposterior and lateral planes.
