Abstract

This study was conducted on 72 pediatrics patients (< 18 y) , they were admitted to pediatric ICU in Assiut university pediatric hospital from March 2021 to March 2022.The study was carried out in Microbiology unit, Clinical Pathology Department at Assiut University Hospital .
This study was trying to identify pathogens causing lower respiratory tract infection among studied patients, detect antibiotic resistance by routine culture method and biofire filmarray pneumonia panel plus, evaluate the performance of the BF-PP as a rapid accurate tool.
All study group patients were subjected to full history taking including Time of admission age, sex, weight, diagnosis on admission, on mechanical ventilation or not, duration of mechanical ventilation, radiological signs of chest infection and clinical data suggesting presence of HAP/VAP .
Bronchoalveolar lavage (BAL) samples were collected from pediatric patients who were suspected to acquire hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP) .
Microbiological diagnosis were done by Routine conventional methods which included: microscopic examination ,routine culture method , identification and antibiotic susceptibility testing of isolated organism were done according to manufacturer instructions by VITEK 2 Compact .all accepted BAL samples were tested with BF-PP.
The result revealed that bacterial and viral infections were common causes of VAP/HAP among pediatric patients in PICU, the most common detected bacteria was klebseilla pneumonia ,while most common detected virus was Rhinovirus.
Yeast was detected by the culture method in 29 patients with predomi-nance of Candida Spp.
We reported a high percentage of antibiotic resistance, the most prevalent resistant gene in our study were CTX-M and NDM genes.
The Film Array pneumonia panel offers a rapid and sensitive diagnostic method for lower respiratory tract infections. However, clinical correlation is advised to determine its significance in interpreting multiple pathogens and detection of genes involved in antimicrobial resistance.
