Abstract

Aim: The purpose of this study was to evaluate the marginal discrepancy and clinical
evaluation of endocrowns prepared with ferule preparation design compared to that prepared
with butt joint preparation design, manufactured by heat pressed lithium disilicate at 0, 3, 6, 9
and 12 months follow up periods.
Methodology: A total of 26 patients (10 females and 16 males) were selected for this study with
an age range of 21-50. They were divided into two groups (G1&G2) according to the preparation
design (N=13). First group received butt joint prepared Endocrowns over posterior teeth (molars).
Second group received circumferential ferrule prepared Endocrowns over posterior teeth (molars).
Both groups where fabricated using heat pressed lithium disilicate. Before cementation of the
restorations a replica where constructed using light and putty additional silicone for the assessment
of the marginal discrepancy and internal fit. Cementation was done according to the manufacturer
instructions in every single case with the same operator to ensure the standardization of the
procedures. Endocrowns of both groups where assessed at the delivery appointment for clinical
performance according to the modified USPHS criteria and the follow up continues every 3
months for 1 year.
Results: The marginal and internal gaps of both groups were within the clinical acceptable range,
but the ferrule group, recorded statistically significant higher internal gap mean than the butt
joint group. Regarding the clinical performance all restorations showed 100 % alpha and there was
no statistically significant difference between both groups for all tested outcomes. (Marginal
adaptation, retention, fracture, marginal discoloration, secondary caries and periodontal health)
over a period of one year.
Conclusions: Endocrowns restoration designed with butt joint preparation revealed better internal fit than
those designed with circumferential ferrule. There were no difference in the clinical marginal adaptation of
endocrowns prepared with butt joint and circumferential ferrule preparation designs. The marginal and
internal fit of both butt joint and circumferential ferrule preparation designs of endocrowns, shows
clinically acceptable range. Both designs (butt joint and circumferential ferrule) showed high successful
clinical performance in terms of marginal adaptation, retention, fracture and periodontal health over a
period of one year.
