Abstract Covid 19 is a disease caused by a virus belonging to SARS Co V2 family of viruses. It primarily effects the lungs resulting in inflammation and pneumonia. Mild cases can be managed at home while moderate and sever cases is admitted to hospital and given supplemental oxygen along with other treatment modalities. Corticosteroids are low-cost anti-inflammatory drugs that mimic cortisol, the hormone naturally produced by the adrenal glands in humans. Dexamethasone is long acting corticosteroids that decreases inflammation by suppressing migration of polymorphonuclear leukocytes (PMNs) and reducing capillary permeability; stabilizes cell and lysosomal membranes, increases surfactant synthesis, increases serum vitamin A concentration, and inhibits prostaglandin and proinflammatory cytokines. Methylprednisolone Controls or prevents inflammation by controlling rate of protein synthesis, suppressing migration of polymorphonuclear leukocytes (PMNs) and fibroblasts, reversing capillary permeability, and stabilizing lysosomes at cellular level. Summary of our results: Regarding PaO2/FIO2 there was no statistically significant difference between the two studied groups. In terms of PaO2, it was significantly higher in dexamethasone group than methylprednisolone group at days 1, 2, 3 and 4 while at day 7, it was significantly decreased in dexamethasone group than methylprednisolone group. In dexamethasone group, there was a statistically significant difference in terms of PaO2, CO2 and HCO3 between different time periods while PaO2/FiO2 was comparable between different time periods. Whereas methylprednisolone group , there was a statistically significant difference in terms of PaO2/FiO2, PaO2 and CO2 between different time periods and no significant difference was detected regarding HCO3 between all time periods. Regarding CRP there was statistically significant difference between the two studied groups in Day 3, Day 5 and Day 7.There was no statistically significant difference between the two studied groups in Day 1. There was no statistically significant difference between the two studied groups regarding ferritin and LDH. Regarding IL6 at day 3 there was significant increase in DEX group Acompared to MP group D dimer at day 1 in DEX group was higher than MP group while at day 3, day 5 and day 7 there was no statistically significant difference between the two studied groups as regard D dimer. There was highly statistically significant difference between the different periods in both groups regarding ferritin, LDH, IL-6 and D dimer. Regarding procalcitonin at day 1, 5, 7 there was a significant increase in group B compared to group A.There was no statistically significant difference between the two studied groups at day 3. Regarding vasopressor and mechanical ventilation there was no statistically significant difference between the two studied groups. The duration of stay was higher in patient received dexamethasone than patient received methylprednisolone As regard 28-day mortality there was no statistically significant difference between the two studied groups. |
