Abstract

Reconstruction of oral and maxillofacial defects is demanding procedure, and this can be achieved using different local, regional, and free flaps. SMAF has many merits over other regional flaps especially in reconstruction of intraoral defects and this is due to Its excellent color and texture match.
Despite these characteristics Its use is still limited by the short pedicle. Its arc allows coverage of the lower two thirds of the face. So, in our study we elucidated feasibility and significance of pedicle lengthening for the benefIts of this procedure for reconstruction of different maxillofacial defects.
Aims
To assess the feasibility of lengthening the pedicle of SMAF at various levels up to facial artery for reconstruction of indicated orofacial surgical defects.
Results
-Pedicle length increase significantly between different levels with P value <0.001, with the higher value obtained after dissection at mylohyoid perforator followed by dissection to the level of external carotid artery followed by dissection at submandibular gland level.
- Nine cases were successful, and one case was failed due to ischemia.
- Clinical monitoring values of flap (color, capillary refill, surface texture, and doppler sound) enhanced significantly toward normal values from first day up to one week postoperatively in most cases.
