Abstract

In short, we believe that the method of anatomical repair with the technique of separating the muscles without stretching the abdominal wall simplified and helped a lot in the treatment of large and complex abdominal defects. In this type of repair, we use the body’s own tissues, and this avoids us from using the side effects of external grafts.
We can use this type of repair only when the mid and longitudinal muscles are available and the nerves that supply them are available.
And the function of this muscle was preserved in this way of repair. We strongly support the use of this method in the first place due to the use of the body’s own tissues in the repair of large and complex abdominal defects.
Large and complex abdominal wall hernias present a major challenge for surgeons. This causes many serious complications. Surgery is the only solution for repair and treatment. And treatment should be the least risky and feasible, and the least injury to hernia recurrence.
There are many factors that lead to the occurrence of hernias in the abdominal wall in large sizes. This may result from the non-fusion of the wound immediately after immediate surgeries, which leads to the opening of the wound or the lack of fusion. Oan leads to a chronic incisional hernia after a period of surgery.

The repair using the body’s own tissues, which involves the grafts of the femoral, or muscular, or muscular-facial, or both, is constantly increasing. In addition to the strength factor in muscle repair, the use of the original tissues of the body gives the necessary blood vessels at the site of repair. These blood vessels provide resistance to infection by providing inflammatory cells, nutrients, and oxygen. The musculoskeletal technique is used to free a portion of the abdominal muscles as a flap to cover the defect.
The goals of abdominal wall reconstruction are to protect and cover the abdominal viscera, to restore continuity of structure and function of the musculoskeletal system, to prevent recurrent abdominal hernias, and to achieve an acceptable surface abdominal wall.
In this study, we used the method of repair by separating the muscle components to free a part of the abdominal muscles, as it was used as a flap to cover the defect, with reinforcement with a non-self surgical mesh. This combination of the two techniques by placing the surgical mesh over the place of repair significantly reduced the recurrence of hernia with an acceptable rate of complications.
This study was conducted on twenty patients with large abdominal hernia. After studying their age, medical condition, weight, cause and size of the hernia. The level of complications was studied in terms of the rate of recurrence with hernia, inflammation, local swelling, hematoma and infection to see if this combination of component separating technique and reinforcement with a surgical mesh may result in a decrease in the level of complications. He also mentioned the period of stay in the hospital after surgery. In this study, we used the method of repair by separating the muscle components to free part of the abdominal muscles, as it is used as a flap to cover the defect with reinforcement with a non-self surgical net. This combination of the two techniques by placing the surgical mesh over the place of repair significantly reduced the recurrence of hernia with an acceptable rate of complications.
In conclusion, this study has proven that using the technique of separating the muscular components to free part of the abdominal muscles, as it is used as a flap to cover the defect with reinforcement with an insoluble surgical mesh, is the optimal treatment for complicated abdominal hernia cases, as it is the least in the rate of recurrence of hernia in the presence of a low level of complications rate, The technology is easy to use and inexpensive compared to other technologies.
