Abstract

Fourteen completely edentulous patients will be selected from those attended to the outpatient clinic of the Prosthodontics Department, Faculty of Dentistry, Tanta University for each patient, conventional acrylic complete denture was constructed. The mandibular denture was prepared as a radiographic template for CBCT (cone beam CT) then the stereo lithographic surgical template was printed by 3D printing technology. For all patients, one stage surgery was done in the two groups. Two vertical implants (12 mm length and 3.5 mm width (narrow plate form) were inserted in the canine areas bilaterally then healing abutment were screwed into the implants. After that telescopic attachment were designed as fellow for the 2 groups group A (GA): patient who received implant retained over denture with PEEK patrix and matrix. group B (GB): patient who received implant retained over denture with PEEK patrix and Titanium matrix. The same parameters for designing primary copings were maintained for all groups concerning 5mm height (2mm gingival height was paralleled and the occlusal 3mm was occlusally tapered 4o). The computer numeric control (CNC) data were transmitted to a milling machine and connected to the CAD system (Sheraeco_scan3 Germany) to mill primary copings from PEEK. the secondary copings on their 3D image. Parameters used for designing secondary copings were parallel walls with minimal wall thickness of 0.5 mm and an occlusal space (0.3mm) was preserved between the primary and secondary copings. Projections were added to secondary copings design to enhance their mechanical retention to the over denture fitting surface. Data were finally transferred to the CAM program for milling of the secondary crowns from PEEK (ceramic filled Bio HPP) for the (A) group of patients and from titanium for the (B) group. For all patients the direct pick-up of peek matrix (For group A) and titanium matrix (for group B) were done then Clinical, retention and radiographic evaluation were carried out at time of implant loading, After 6 months. And After 1 year Modified Bleeding index: there was no significant difference in comparing the durations throwing groups and there no significant difference for comparing between groups in each duration. Plaque index: there was no significant difference in comparing the durations throwing groups and there no significant difference for comparing between groups in each duration. Modified Gingival index: there was no significant difference in comparing the durations throwing groups and there was no significant difference for comparing between groups in each duration. Modified Propping depth index: there was significant decrease in comparing the duration throwing group for two groups and there’s no significant difference in comparing between groups in each interval. For radio graphical Bone marginal loss: there was highly significant difference in comparing the duration throwing group for peek group and highly significant difference for titanium group and there was no significant difference for comparing between groups in each interval. Bone density: there was highly significant difference in comparing the duration throwing group for peek group and highly significant difference for titanium group and there was significant difference between groups at 6months and 12 months but there is no significant difference between groups at insertion for comparing between groups in each interval. For retention evaluation there was highly significant difference decrease in comparing the duration throwing group for peek group and highly significant difference decrease for titanium group and there was highly significant difference in comparing between groups in each interval.
