Abstract

Objective: To evaluate the efficacy of intraoral drainage of isolated
submandibular space abscess as a minimally invasive surgical technique
compared to the standard trans-cervical approach.
Patients and methods: Forty individuals with a unilateral submandibular area
abscess were enrolled in this prospective study. They were randomly divided
into 2 equal groups; trans-cervical surgical drainage (group A) and intra-oral
surgical drainage (group B). Data were analyzed, including demographic
information, the need for 2nd drainage, the length of postoperative stay in the
hospital, the development of scars, and potential complications.
Results: Intraoral drainage (group B) reduced the mean operative time by
15.25 minutes (P<0.001) compared with trans-cervical incision (group A). In
terms of postoperative hospitalization, there wasn’t any major difference
between the two groups. In both groups, no patients showed marginal
mandibular nerve paralysis. Three patients only have cervical scar in group
(B) who required external drainage due to recollection. No recurrence was
detected in group (A).
Conclusion: This study reveals that isolated submandibular abscess can
sometimes be successfully treated with intraoral drainage, which is more
effective than trans-cervical surgery in terms of cosmetic results and shorter
recovery times.
