Abstract

Summary Surgical decompression is an effective method for treatment of CTS. It aims to divide the TCL, thereby increasing the canal volume and reducing the pressure on the nerve without damaging the adjacent structures. Many different methods have been described for carpal tunnel release, including the classic open approach through conventional interthenar longitudinal incision (beginning at Kaplan8s cardinal line distally, may be extended proximally beyond the distal wrist crease), endoscopic approach, and more recently, the limited incision and mini-incision techniques including their variations. Conventional open approach remains the gold standard for the surgical treatment of CTS. It is safe, relatively inexpensive and has the advantage of wide exposure and direct visualization of vulnerable anatomical structures throughout the procedure. Some complications such as pillar pain, scar tenderness and delayed return to work were associate long learning curve with increased rd with this approach. Endoscopic techniques: one-portal and two-portal endoscopic approaches offer some advantages such as small scar, less postoperative pain, early return to work and preservation of grip strength. However, they require costly equipments, technical support, prolonged operative setup time and isk of damage to the adjacent tissues and incomplete cutting of the TCL. More recently, various open mini-incision techniques as; limited palmar incision, double incision of Wilson, wrist mini incision, have been described by either conventional instruments or special instruments trying to combine the simplicity and safety of
