Abstract

The present study is a fifteen years retrospective, descriptive study aimed to assess the flow rate, clinical presentations, endoscopic pictures of gastritis, and outcome of children attended GIT Endoscopic Unit, Assiut University Children Hospital (AUCH) in the period from January 2005 up to December 2019.
The main finding of the current study:
- The prevalence of gastritis during the study period was 9.4%.
- The median age of the studied participants was 4 years and ranged from 3 months up to 17 years old.
- More than half of the studied gastritis cases were less than five years old.
- No male sex predominance.
- Persistent vomiting and gastrointestinal bleeding were the commonest complaint among our studied cases.
- The prevalence of H. pylori infection, non-specific gastritis, and eosinophilic gastritis diagnosed by histo-pathological picture via gastric biopsy among pediatric patients presenting with GI complaints in the current study was 54.8%, 24.8%, and 20.3%.
- H. pylori induced gastritis was more frequent among older aged children, mainly presented with GI bleeding, showing mainly erythematous hyperemic gastric mucosa and a notable nodularity on endoscopic examination, and associated with high recurrence rate.
- Non-specific gastritis was more frequent among younger aged children, mainly presented with persistent vomiting, showing mainly erythematous gastric mucosa on endoscopic examination.
- Eosinophilic gastritis was more frequent among younger aged children, mainly presented with persistent vomiting, showing mainly erythematous gastric mucosa and a notable nodularity on endoscopic examination, and associated with lower recurrence rate.
from this study we concluded that: Gastritis represents only 9.4 % of children who indicated for Upper GI endoscopy. The median age of the studied cases was 4 years (range; 3 months - 17 years) with no sex predominance; also studied cases were mainly persistent with vomiting. H. pylori was the commonest cause with high recurrence rate. Higher recurrence was also observed among older aged patients, those presented with recurrent GI bleeding, and erythematous hyperemic mucosa on endoscopic examination.
Based on this finding we recommend that larger prospective multicenter surveys were needed to accurately determine the prevalence of gastritis and its possible causes among Egyptian children, adapt continuous screening program for all children suffered from recurrent GI complaints, with careful history taking to be able to detect the cause of gastritis for proper diagnosis and treatment to reduce their recurrence rate.
