Abstract

Objective: To compare the depth of the corneal demarcation line in transepithelial versus epithelial-off collagen cross linking in keratoconus using anterior segment optical coherence tomography. Design: Prospective, interventional, non-randomized, non-controlled study. Materials and Methods: 40 eyes of 20 patients were divided into 2 groups. For each patient, the eye with the more advanced KC stage received epithelium-off CXL (the intended power is set to 30mW/cm2 and irradiance is performed for 4minutes, giving a total energy of 7.2J/cm2). The other eye received transepithelial CXL (UVA irradiation is performed, using Avedro KXL CrossLinking System, for 2.40minutes, with an intended power of 45mW/cm2, giving a total energy of 7.2J/cm2). Corneal stromal demarcation line was measured, using Anterior segment OCT (Topcon 3D OCT-2000) one month postoperative for both eyes. Results: The demarcation line was patchy in 50% of the transepithelial eyes; the other half showing a demarcation line. However, there was a demarcation line in all epithelium-off eyes. There was a statistically significant difference in the incidence of a patchy corneal demarcation line between transepithelial and epithelium-off techniques. P value = 0.001 (p value < 0.05).Mean depth of corneal demarcation line was 183æm in transepithelial technique and 219æm in epithelium-off technique. There was a statistically significant difference in corneal demarcation line depth between transepithelial and epithelium-off techniques. P value = 0.008 (p value < 0.05)
