Abstract Background:Although it is known from the beginning of the last century that splenectomy increases the risk of infections, however for many decades it has been the preferred treatment for traumatic injuries. Compared with adults, splenic injury in children is less likely to require operative intervention. Non-operative management of splenic traumatic injuries is to be preferred in order to leave the immune function of this organ untouched. Objective:The purpose of this study was to assess the rate of success and complication of the conservative management in splenic trauma at Kasr Al-Ainy emergency hospital. Methods:ninety three patients presented with splenic traumafrom August 2017 until March 2018 at Kasr Al-Ainy emergency hospital. All patients underwent an initial assessment upon arrival to the emergency room using ATLS protocol. Subsequent diagnostic procedures were utilized based on the hemodynamic stability of patients. Hemodynamically stable patient should receive an abdominal CT scan with contrast, he will be admitted to emergency department for conservative management. Hemodynamically unstable patients unresponsive to intravenous fluids and intensive support. If the abdomen is the obvious area of blood loss and confirmed by FAST, urgent laparotomy and Splenectomy was done. Results:the mean age of the study group was 16.9±14.9 years with a male gender victims forming 75.3% of the studied group. 60 cases were managed non-operatively. Operative management was done in 33 cases. Splenectomy was done in 25 cases while splenic salvage technique was applied in 8 cases |
