Abstract Objective This study was performed during a 2 years duration from March 2014 to March 2016. It aimed at reviewing and evaluating the efficacy of the 1992 updated Jones criteria for the diagnosis of rheumatic fever (RF) in Egyptian children, determining the exact frequency of each criterion, detecting the efficiency and the unusual constellations of the various criteria and accurately describing the major criteria in our in Egyptian patients with RF. Also outlining the role of echocardiography in the diagnosis of RF and proposing local Egyptian guidelines for diagnosis of RF in the Egyptian setting possibly incorporating the echocardiographic criteria to improve the sensitivity of the Jones criteria in an endemic setting. Methods .A retrospective study including statistical analysis for data that had been previously collected at RF clinic at Cairo University Specialized pediatric Hospitals (CUSPHs) and including either known RF cases following up at our clinic or newly diagnosed cases presenting to our clinic during the period of study. The study included 891 Egyptian children aged from 5- 15 years diagnosed according to modified Jones criteria (1992) into either proved RF who completely fulfilled modified Jones or probable RF with incomplete fulfillment of Jones yet mostly RF according to our physicians. Univariate, bivariate and multivariate analysis were done. Discriminant analysis technique was used to develop the clinical prediction model. Sensitivity, specificity and overall prediction accuracy for the prediction model were calculated.
