Abstract

Abstract
Background: To evaluate the diagnostic value of QTc interval and QT dispesion as a marker of early cardiac affection in beta thalassemia major pediatrics.
Methods:QTc interval and QT dispersion are assesed in children with beta thalassemia major using 12 lead Electrocardiogram (ECG) and to correlate them with various laboratory and echocardiographic data.
Results: 50 patients aged from 2-14 years compared with healthy control group,The QTc interval and QT dispersion on ECG were prolonged with significant difference pattern ( P= ,027),(P=0.059) respectivly.
ECHO showed that 38% of B-thalassemia major patients have pulmonary hypertension and 6% of them have impaired cardiac function.
Many factors were correlated with pulmonary hypertension with significant correlation including age in years with (P =,054) and impaired cardiac function with (P=,022) ,while others had no correlation with PH including serum ferrittin with (P=0.188), iron chelator intake with P=,597 and splenectomy with P=,975.
Serum ferrittin level had no significant correlation with PH ,impaired function , QTC and QT dispersion with (P=0.188) , (P=,439) , (P=,797), (P= ,912) respectivly.
Conclusion: In this study ,beta thalassemia major children compared with controls had difference of QTc interval and QT dispersion that was of statistical significance . Significantly high pulmonary artery pressure was detected in 38% of patients.
Impaired function was significantly associated with PAH while was not significantly correlated with serum ferrittin.
