Abstract

The aim of the study was to assess the accuracy of transverse cerebellar diameter (TCD) and fetal kidney length (FKL) measurements in estimation of the gestational age in the second and third trimesters according to the last menstrual period, compared to BPD, HC, AC and the FL.
This study was conducted on 170 pregnant women at department of Obstetrics and Gynecology, Kasr Alainy Hospital; Cairo University from May 2020 till october2021
The patients were divided into two groups second trimester group and third trimester group, the transcerebellar diameter, the fetal kidney length, biparietal diameter, head circumference, abdominal circumference and femur length were measured for determination of gestational age.
In the present study, the demographic data revealed that the age range of the pregnant ladies was from 19 – 40 years.
Among the studied cases, the mean GA by LMP was 27.79 weeks, whereas the mean measurements of; the HC was 256.83, the BPD was 68.40, the AC was 236.42, the FL was 52.70, the TCD was 32.94 and the FKL was 31.48, all in millimeters.
The measurements of BPD, HC, AC, FL, FKL and TCD in the second trimester when correlated to GA by LMP shows a statistically significant P values, that was <0.001, <0.001, <0.001, <0.001, <0.001, <0.001 respectively.
The measurements of BPD, HC, AC, FL, FKL and TCD in the third trimester when correlated to GA by LMP shows a statistically significant P values, that was <0.001, <0.001,<0.001, <0.001, <0.001, <0.001 respectively.
The measurements of BPD, HC, AC, FL, FKL and TCD in the second and third trimesters when correlated to GA by LMP shows a statistically significant P-values, that was <0.001,<0.001, <0.001, <0.001, <0.001, <0.001 respectively.
The intraclass correlation coefficient of Fetal biometry and TCD when correlated to GA by LMP shows that TCD had intraclass correlation coefficient with a value equal to 0.987 while the FKL has a value equal to 0.928.
Despite the measurements of BPD, HC, AC, FL,FKL and TCD in the second trimester and in third trimester in weeks when correlated to GA by LMP shows a statistically significant P values, that was <0.001, <0.001, <0.001, <0.001,<0.001, <0.001 respectively.
But the intraclass correlation coefficient of Fetal biometry and both TCD and FKL when correlated to GA by LMP revealed that both AC and FL had the highest intraclass correlation coefficient with a value equal to 0.990 with less bias followed by TCD with a value equal to 0.987 then HC with a value of 0.969 then FKL with a value equal to 0.928 and the less accuracy was for the BPD with a value equal to 0.859.
Linear regression analysis to detect accuracy of TCD in estimation of GA when correlated to GA by LMP shows a statistically significant P values in the second trimester and in the third trimesters as follows 0.969 and 0.949 respectively that denotes less bias in measurement and in estimating GA.
Linear regression analysis to detect accuracy of FKL in estimation of GA when correlated to GA by LMP shows a statistically significant P values in the second trimester and third trimester as follows 0.783 and 0.852 respectively that denotes less bias in measurement and in estimating GA.
This study showed that fetal kidney length and TCD increase with an increase in GA and show an excellent correlation coefficient which suggests good agreement and reproducibility of measurements and is not affected by the discrepancy of the late trimester. This study concludes that FKL and TCD show a strong correlation with fetal GA, with a steady growth rate throughout pregnancy.
