Abstract

Despite wide availability of antibiotics and advanced imaging modalities, orbital and intracranial complications of sinusitis still occur with low incidence rate but significant morbidities or even death.
The aim of our study was to define the role of multi detector computed tomography and magnetic resonance imaging in evaluation of different orbital and cranial complications of sinusitis
Our study was carried on 20 patients with signs and / or symptoms of rhino-sinusitis with suspected orbital and intracranial complications based on history and clinical examination and referred for medical imaging, including 11 (55%) male, 9 (45%) female, their ages ranged from 5 to 70 years with mean age of 32.2 and most affected age was 40-70 years.
All patients were subjected to full history taking, clinical and laboratory investigations.
Gadolinium enhanced PNS MRI using (Philips) machine was performed for 14 patients. Images were obtained in axial, coronal and saggital spin echo T1WI and T2WI with axial FLAIR cut for brain scan followed by DWI which was obtained with single- short spin echo planar in axial plane. Following diffusion, contrast enhanced MRI sequence was obtained in axial and coronal planes then reconstructed in three orthogonal planes with fat suppression.
Complementary non contrast CT was performed for seven patients out of 14 cases who had MRI for better assessment of bony anatomy of PNS.
