Abstract

The congenital anomalies and structural disorders of the child organs are one of the dangers influencing their wellbeing. CHD is the structural heart and large vessels abnormalities that are most affecting in heart function. HRQOL is a quantification of the influence of a known illness, therapy, or health policy on the ability of the individual patient to both function in and derive personal satisfaction from various physical, psychological, and social life contexts. Mortality in children with CHD diminished significantly over the past decades. However, those survivors are at risk for decreased HRQOL which occurs under the influence of many variables such as environmental, parents stress, depression of children, and any surgical intervention. This work aimed to assess the impacts of congenital heart disease in children and adolescents on the health-related quality of life (HRQOL) including emotional, psychological, learning, educational, and activity of these children and adolescents. Our study documented significant HRQOL deficiencies among children and adolescents with CHD in comparison to their normal counterparts. We conducted this cross-sectional study throughout the year 2020 on five hundred (500) participants ; 250 patients aged around two to Eighteen years with CHD who were without any interventions, after cardiac surgery, catheter intervention, or palliative procedure and 250 healthy control group to evaluate their health-related quality of life outcome. We collected the data by a convenience sample method using the PedsQL 4.0 Generic Core modules for children/ adolescents and their parents were used to collect HRQOL data to assess the impacts of congenital heart disease in children and adolescents on the HRQOL. This cross-sectional comparative study was carried out in Mansoura University Children’s Hospital “MUCH”, Mansoura Insurance Hospital, Damietta General Hospital, Kafr el-sheik University Hospital, Zagazig University Hospital, and Tanta University Hospital. Most children with CHD were males 55.2%, with a median age of 9 years, 76.0% of them live in a rural area. 51.6% were born to parents with high school. There were 64% with positive family history and 63.2% with negative consanguinity. There was a significant decrease in HRQOL among children with CHD as compared to healthy children. The most frequent acyanotic CHD in our study was VSD (29.6%) followed by ASD (17.2%), and PDA (15.3%). The most frequent cyanotic CHD in our patients was TGA (38.3%) followed by TOF (29.8%), and DORV (17%). Conclusion : Children and adolescents with CHD may have an aspect of reduction of HRQOL as compared to healthy ones. Also, there is improvement in HRQOL scales; physical, social, and school performance after interventions whether surgical repair, catheter intervention. Physical health and school functioning were affected more in cyanotic patients.
