Abstract

Camptodactyly remains one of the most challenging aspects of plastic surgery. Considerations of full correction of contracture , full ROM, less invasive manouvers when trying to manage camptodactyly .
Various repair methods including conservative management such as splintage by different types of splints and different durations of treatment and too many trials for surgical intervention either for the skin ,lumbrical , extensor mechanism FDS tendon transfer , pulley release +\ - pinning reported to be used in the repair of camptodactyly.
Our idea was the first of it’s kind through release of the camptodacytyly through K – wiring only
The present work included 12 cases with 20 digit complaining of camptodactyly presented in The Plastic Surgery Department, Minia University Hospital in period between October 2019 till October 2020.
Their ages ranged from 3-18 years old with an average age 8.7 years. Five of them were males and seven were females. The type of camptodactyly was type I in 9 patients and type II in 3 patients and no case of type III .
Almost affected digit was the little finger ( 15 digit) and only 4 digits of our study were ring and only 1 time middle finger .with variable degrees of contractures ranging from 40 – 60 degrees with mean degree of contracture 49.5 degree.
The protocol of management of camptodactyly in this study included K – wiring after Gradual blunt release of the finger till being straightened under anesthesia was done under vision .
According to results 25% of patients had Good results, 33% had Excellent results with excellent outcome, only 17% of cases had fair results with bullae formation and recurrence at late follow up , 3 digits needed second stage of wiring and only one of them succeded by rewiring and the other 2 digits slipped again and (25%) had poor results due to failure (slippage of the wire ) .
Patient satisfaction was tested regarding release of the contracture , functional outcome , ROM and their opinion about short hospital stay and about overall results of operations.
