Abstract

This case-control study was conducted in the Sleep Disordered Breathing (SDB) Unit of the Chest Department at Assiut University hospital, one of the oldest units in Upper Egypt specialized in diagnosis and treatment of sleep breathing disorders. The study was approved by the Scientific Ethics Committee of Faculty of Medicine of Assiut University and informed consent was obtained from the participants to deal with their data for scientific purposes. The current study included 50 participants during the period from January 2018 to July 2019. Out of the studied subjects; 30 (60%) cases with OSA as patients group while the other 20 (40%) subjects were healthy and enrolled as control group. Among the patients group, 11(36.7%) patients had moderate OSAS and 19 (63.3%) patients had severe OSAS. The aim of this study was to assess the risk of hypercoagulability in patients diagnosed with OSA, by measuring the levels of some factors in the blood that may indicate an increased risk of coagulability with a comparison of results between patients and control. Moreover, study the relationship between the level of these factors and the degree of severity of the disease. These factors included complete blood count (CBC) indices, blood coagulation tests, C - reactive protein, d- dimer, and lipid profile. At the end of the study, we reported the following results: 1- OSA patients had significantly higher red cell distribution width (RDW) compared to the control group. Moderate OSA and severe OSA patients had insignificant differences as regarding RDW. There was insignificant positive correlation between apnea-hypopnea index (AHI) and RDW and between oxygen desaturation index (ODI) and RDW. 2- OSA patients and healthy control had insignificant differences as regarding hematocrit (Hct) value. Patients with severe OSA had a significantly higher Hct compared to those with moderate OSA. An insignificant positive association was found between the AHI and Hct value, and between ODI and Hct value. 3- OSA patients had a significantly higher mean platelet volume (MPV) compared to the control group. The comparison between moderate and severe OSA patients showed insignificant differences with regard to MPV. An insignificant positive association was found between the AHI and MPV, and between ODI and MPV. 4- OSA patients had a significantly higher platelet distribution width (PDW) compared to the control group. The comparison between moderate and severe OSA patients showed insignificant differences with regard to PDW. A significant positive correlation was found between the AHI and PDW while the association between ODI and PDW was positive but insignificant. 5- OSA patients had a significantly higher platelets count compared to the control group. The comparison between moderate and severe OSA patients showed insignificant differences with regard to platelets count. A significant positive correlation was found between the AHI and platelets count while the association between ODI and platelets count was positive but insignificant. 6- Both OSA patients and healthy control groups had insignificant differences as regarding prothrombin time (PT), international randomized ratio (INR), and activated partial thromboplastin time (aPTT). Moderate and severe OSA patients had insignificant differences as regarding these coagulation markers. It was also observed that AHI and ODI had insignificant negative correlation with these markers. 7- OSA patients had significantly higher CRP compared to the healthy control. Both moderate and severe OSA groups of patients had insignificant differences as regarding CRP. It was also observed that AHI and ODI had insignificant positive correlation with CRP levels. 8- OSA patients had significantly higher d-dimer level compared to the control. Moreover, severe OSA patients had significantly higher d-dimer compared to moderate OSA patients. Both AHI and ODI had insignificant positive correlation with d-dimer. 9- In our study, it was noticed that both OSA patients and control groups had insignificant differences as regarding triglycerides (TG), high density lipoproteins (HDL) and low density lipoproteins (LDL). Both moderate and severe OSA patients had insignificant differences as regarding TG, HDL and LDL. Moreover, insignificant correlations were found between AHI and ODI with the lipid profile.
