Abstract This study attempted to investigate the effects of micronutrients on the clinical outcomes of ACG regarding soft tissue healing, recurrence of ONF and preservation of the grafted bone. The micro-nutrients that were investigated are iron profile, Copper, Zinc and A/G ratio. The study included 22 patients with Alveolar Clefts, grouped into 2 groups: Retrospective group Ind Prospective group. The retrospective group included 11 cases and the prospective group included 11 cases done starting from January 2016 till December 2018 at Cleft Care Clinic with the routinely performed procedure using an iliac crest graft, where ACG was performed using standard procedures. The nutrition of the prospective group only was assessed and followed up pre-operatively by labs including Iron profile, Copper, Zinc and A/G ratio. Assessment of the results was based on both clinical and radiological evaluation. The clinical evaluation addressed the healing of the soft tissues around the graft, the recurrence of ONF, the presence of any signs of infection and the presence of wound dehiscence. The radiologic evaluation was based on Bergland scale. Radiographs were made pre-operatively, 6 months and 1 year postoperatively. Normal soft tissue healing was evident in patients of both groups except in 7 patients. Those patients suffered from wound dehiscence, four of them experienced recurrence of ONF. Radiographically, comparison between the 2 groups was done using Bergland scale where 11 patients out of the 22 showed successful bone grafting and therefore no more surgeries regarding the grafting. In the sample included, the overall success rate in our study according to the studied parameters was: • Prevention of ONF recurrence = 81.18% • Decrease in wound dehiscence = 60.00 % from the results of this study and within its limitations, it could be concluded that: 1. Attention to micronutrition whether by food or supplementation has a direct effect on the reduction of pre-existing ONF recurrence and the rate of wound dehiscence. 2. Micronutrients assessed in this study has a significant effect on the graft healing radiographically. 3. We consider this study as a base line audit on the effect of nutrition in patients with CLP especially in ABG. This could set the platform for future research in this mostly overlooked important direction. |
