Abstract

Introduction: Psychoactive substances are believed to be aphrodisiac; but in reality, they have deleterious effects on all the aspects of sexual function. These substances may exert their inhibitory effect on erection through their effects on central neurotransmitter pathways (serotogenic, adrenergic or dopaminergic). The Aim of this study: 1- To evaluate the risk of sexual dysfunction in patients with opiates abuse disorder during the use and three months after abstinence. 2- To compare and detect if there a difference in sexual dysfunction between tramadol HCL dependence and heroin dependence. Materials and methods: The study comprises 42 male patients with opioid dependency (21 heroin and 21 tramadol HCL use disorders patients) and 42 healthy volunteers, presenting to the both outpatient clinics and the inpatient ward of the Addiction Unit of Psychiatry Department, Mansoura University Hospital. Results: 1-Regarding Sexual dysfunctions across the two groups of opiate dependents, in comparison with healthy volunteers, the finding of high prevalence of SD in opioid use disorder patients (both groups) compared to healthy volunteers, suggests that active use of opioids is associated with significant SD. 2- Regarding sex hormone levels and their correlation to sexual dysfunction, the present study found a significant correlation between the hormonal profile and the sexual functions of both subjects. 3- Regarding heroin use disorder patients, there was a significant correlation between the free testosterone level and the duration of substance use in heroin use patients. A significant correlation was found between duration of use of heroin and sexual functioning. 4-Regarding tramadol HCL use disorder patients, there was no significant correlation between the free testosterone level and the duration of substance use in tramadol use patients. However, a significant correlation was found between duration of use of tramadol and sexual functioning. 5-Regarding both heroin and tramadol use disorder groups, no significant correlation between the daily dose and the sex hormone levels or sexual functions scores were observed in this study. 6- An improvement in sexual function three months after abstinence of opioids were observed among patients.
