Abstract Residual adduction of clubfoot is very common residual deformity of clubfoot.this deformity cause gait problems shoe fittig and also normal activity of life may be restricted and exacerbated by prescence of pain in some cases. Soft tissue release only cant solve this deformity,and also the children are too young for other salvage procedures like triple arthrodesis.. The cause of residual deformity comes back to residual metatarsus varus or residual subluxed talo navicular joint In this study we aim to evaluate and manage such residual deformity with double column osteotomy (shortening of the lateral column of the foot and lenghthing of the medial column of the foot).i addition soft tissue realease (plantar fasciotomy and abductor hallucis release). 30 cases were analyzed clinically and radiologically, in age between 3 -11 years old. We analyzed residual adduction of fore foot and also associated condition like flexible hind foot varus and cavus deformity. Xrays in anteroposterior view and lateral view were obtained. All 30 cases underwent double column osteotomy and soft tissue release,four cases have cavovarus deformity which was managed by soft tissue release. Evaluation system was obtained depending on clinical and radiographic parameters. clinical parameters like absence of adduction ,varus cavus and shoe fitting ,parents satisfaction. |
