Abstract

The aim of this study was to evaluate the role of dynamisation of interlocking nail tibia on union of closed non comminuted tibia fractures and to determine its complications. The study included 30 patients presented in emergency department in Tanta University Hospital. Those patients were randomly allocated to two equal groups of 15 patients each according to be dynamised or not. Patients in this work, the mean age was 34.5 years. there were 27 males (90%) and three females (10%). The inclusion criteria were recent fractures (within 2 weeks), Patients older than 18 years old, Patients with fractures types A1, A2, A3, B1, B2, and B3 on the (AO) classification. The exclusion criteria included Patient younger than 18 years old, open fracture, fractures types C1, C2, and C3 on the (AO) classification, pathological fractures, neurovascular injury, Fractures which need open reduction such as irreducible fractures. Fractures were fixed with interlocking nails. Weight bearing was encouraged by using walking aids if there is no pain while standing. Assessment of the results was done by two methods radiological and by using functional subjective scale. Radiological assessment was done by comparing time of union between the two groups and Dynamisation was found to have a statistically significant better effect on the time of union, with union time ranged from 12 to 19 weeks with an average of 15.5 weeks. This result was found in non-comminuted fractures type A on the AO classification. Functional assessment by the LEFS scoring system showed that 11 patients (36.67%) had excellent function, 12 patients (40%) had good function, 5 patients (16.67%) had fair function and 2 patients (6.67%) had poor function. Complications in this study ranged from superficial skin infection in 4 patients (13.3%) and numbness due to nerve injury in one patient (3.3%), angular deformity in 2 patients (6.6%) to nonunion in 2 patients (6.6%).
