Abstract Background: Conventional parameters in Right heart catheterization (RHC) for diagnosis of pulmonary arterial hypertension (PAH) represent static components and ignore pulsatile components which are pulmonary arterial compliance index (PACI) and pulmonary stroke volume index (PSVI). The pulsatile components can provide prognostic information in PAH. Aim of the Work: To study the role of pulsatile hemodynamics in assessing the severity and reversibility of pediatric PAH in CHD. Patients and Methods: A prospective study conducted at Ain Shams University hospitals, Pediatric Cardiology Unit, Children’s hospital and Cardiology department from January 2016 till January 2018. 25 patients underwent RHC to assess PAH and were divided into 2 groups, PAH-CHD group and PAH-CHD with S/P PAB & Glenn group. The two groups were furtherly subdivded according to vasoreactivity test (VR test). Results: Patients with positive VR test showed increased PACI and PSVI on applying oxygen yet with insignificant difference (p>0.05).In PAH-CHD group, PAC showed a negative significant correlation with mean pulmonary artery pressure (mPAP) and pulmonary vascular resistance index (PVRI) before and after VR test (p<0.05). Conclusion: PACI may be a predictor for reversibility of PAH in PAH-CHD patients, therefore can be of complementary value in assessing disease severity in pediatric PAH. |
