Abstract

Background, aims: The aim of our study was to assess the expression of metalloproteinase-2 (MMP-2) in chronic and aggressive type periodontitis before and after scaling and root planning. (clinical and immue –histochemical study). And correlate it with clinical parameters in healthy and diseases gingival specimens. Subjects and Methods: Gingival biopsies were taken from three study groups (20 patient diagnosed with chronic periodontitis, 10 patients with Aggressive periodontitis (localized or generalized), 10 patients with clinically healthy gingiva). Plaque index, gingival index, pocket depth, clinical attachment level and bleeding on probing index were documented, before and after treatment and biopsies of diseased groups and control group, were processed for H&E stain and immune stain for MMP-2. Results: we reported the expression of MMP-2 in all base line specimens included in our study. (meanºSD: CP group 2.70± .47, AP group 2.80± .42, and control group .89± .33). No significant difference was found between CP and AP group ( P1=0.8), a highly significant differences were reported when each diseased group was compared with control group ( P2 and P3 = <0.001). The post treatments biopsies of diseased group exhibit great reduction in the level of MMP-2 comparing to pretreatment biopsies of same groups ( meanºSD: 1.50± 50 for CP, and 160±52 for AP) and there was a highly sgnificant difference( Ps = <0.001). Athough no significant difference reported between diseased post treament speciment, we found a significant difference when we used one way test for all groups(p#= 0.009). Pathological specimens, from sites that histologically exhibit signs of chronic inflammation and connective tissue remodeling, revealed that the expression of (MMP-2) was mainly confined to fibroblast cells. Conclusion: Our findings reveal significant differences in the expression of MMP-2 between healthy and diseased groups, reflecting the great heterogeneity in the periodontal status of individuals. A direct correlation is observed between the clinical parameters and expression of MMp-2 secreted in the chronic and Aggressive periodontitis. However, they indicate that gingival fibroblasts are an active source of MMP-2 production in response to a periopathogen.
