Abstract Introduction: Carpal tunnel syndrome (CTS) is one of the most common compression neuropathy of the upper extremity caused by an entrapment of the median nerve at the level of the carpal tunnel. Objective: To know the effect of 905 nm Diode Laser Therapy on pain, grip strength and nerve conduction velocity in patients with carpal tunnel syndrome. Methodology:- The study was conducted on 60 patients distributed into two equal groups group (A) received a 905 nm Diode Laser Therapy plus therapeutic exercise (tendon and median nerve gliding exercise & strengthening exercise), group (B) control group received the same therapeutic exercise for 4 weeks, three sessions per week. Result: There was significant reduction in pain intensity, improvement hand grip strength, improvement score of BQ and improvement of motor distal latency and sensory distal latency in both groups but there is more improvement in group (A) than group (B). Conclusion: 905 nm Diode Laser Therapy combined with therapeutic exercise is more effective in treating patients with CTS than therapeutic exercise alone.
