Abstract

This study was performed to compare the systemic inflammatory response following open and laparoscopic surgery in urology by measuring some of the acute phase proteins pre and post-operative.
All patients were evaluated pre operatively by detailed history and physical examination, routine laboratory investigations and imaging including abdominopelvic ultrasonography, CT with contrast and Renal Isotope Scanning in selected cases.
WBCs count, CRP, ESR and IL-6 were measured preoperatively 24 and 48 hours post-operative. Also intraoperative and postoperative clinical data were collected tabulated and analyzed including operative time, intraoperative blood loss, post-operative NPO hours and hospital stay.
The patient were divided into two main categories open and lap each of them containing three subgroups according to the type of operation (Nephrectomy, Cyst deroofing and Orchidopexy)
There was no difference between groups in preoperative laboratory parameters. We found that WBCs count increased in both groups in post-operative day one and day 2 and this increase was significantly higher in open group.
The same occurred with CRP and IL-6 but this time the difference was significantly higher in open group.
Regarding ESR there was no difference between the two groups in the increase that happened in post-operative day one, while the increase in ESR in post-operative day two was significantly less in lap group than that of the open.
We found that there was no difference between two groups in operative time and the intraoperative blood loss was higher in open group. The post-operative NPO hours, the days’ patient stayed in the hospital after the operation and the periods taken till return to work; all were higher in open group.
