Abstract Objective: To assess the accuracy of platelets indices (MPV-PDW-PC) and their changes in predicting the development and severity of pregnancy induced hypertensive disorders. Type of Study: Prospective observational cohort study Study Setting The study was conducted at Ain Shams University Maternity Hospital Study Period 1st of April 2018 through 30th of September 2018 Population Pregnant patients at ≥ 20 weeks gestational age attending antenatal care clinics or admitted from the ER. Results: Mean platelet volume (MPV) varied significantly between the four study groups; being lower in the normotensive and gestational hypertension groups compared to the two preeclampsia groups; and higher in the preeclampsia with severe features group compared to that without severe features A similar pattern was noted for variation in PDW among the four groups; being higher in the preeclampsia with severe features group compared to the other three groups and higher in the preeclampsia without severe features group compared to the gestational hypertension and normotensive groups ROC analysis of the performance of platelet indices in prediction of subsequent development of preeclampsia revealed that PDW greater than 19.9% could predict development of preeclampsia with sensitivity 78.0% and specificity 93.3%; whereas a MPV greater than 9.5 fL could predict the development of preeclampsia with sensitivity 74.6% and specificity 86.6%. Conclusion: The activation of platelets is becoming increasingly incriminated in the patho-physiology of many diseases including PIH.
