Abstract

Summary of the main results:
We included 6 retrospective studies for the comparison of
efficacy, outcomes and costs of transcatheter versus surgical closure of
patent ductus arteriosus in pediatric patients. We intended to address 6
outcomes (success rate , residual shunt, complications, need for blood
transfusion, length of hospital stay and costs). Costs could not be
included in the analysis because of variability in cost currency and
calculations. Costs varied based on treatment location even when
controlling for currency. In addition, different studies included
different items in their cost analysis. Given these factors, it was
difficult to comment on which treatment strategy cost more. Moreover,
indirect and social costs must be taken into consideration , including
the parents’ loss of income and psychological costs. More studies
concerning the costs; including the operation and Length of hospital
stay, device cost, cardiologist, surgeon and anesthesiologist fees, are
needed for further comparison. We could perform Meta-Analysis for 5
outcomes only and we presented the study results for the six outcomes
including the costs.
Success rate:
Data analysis (fixed – effects model) from the six studies,
showed no significant difference between the catheterization groups
and surgical groups regarding success rate of PDA closure. The
success of PDA closure via catheter-based intervention is associated.
