Abstract

Proximal humeral fractures are very common fractures especially in older population after a low energy trauma. These fractures sustain special consideration in treatment and later prognosis especially the shoulder joint function.
The shoulder is an elegant piece of machinery. It has the greatest range of motion of any joint in the body. However, this large range of motion can lead to joint problems.
Various treatment methods have been used, including Kirschner wire
fixation, suture fixation, external fixation, tension band fixation, Rush pin
fixation, intramedullary nailing, and prosthetic replacement.
The traditional open reduction and internal fixation is used to achieve accurate alignment and avoid neurovascular damage. However, extensive soft-tissue dissection can result in iatrogenic neurovascular damage, high incidence of infection, delayed or non union, shoulder stiffness and avascular necrosis of the humeral head.
The aim of this work is to evaluate the results of deltopectoral approach in comminuted fracture proximal humerus (Three to Four parts according to Neer’s classification) by using cemented hemi shoulder arthroplasty .
This study included ten patients, who are suffering from comminuted fracture proximal humerus (Three to Four parts according to Neer’s classification).
The age of patients included in this study ranged from40-69 years
(mean 57.2±6.3 years). There were four males (40%) and six females(60%).
This study included five fractures resulting from road traffic accident (50% RTA) ,two fractures from falling from height (20% FFH) and three fractures from falling on out-stretched hands (30%).
On admission all patients were assessed by history taking, clinical
examination and radiological assessment.
All patients were treated by deltopectoral approach using the cemented hemi shoulder arthroplasty . Post-operative care consisted of Abduction splint in 20° Abduction and Neutral rotation of the arm , analgesics, anitedematous, antibiotics medication and passive movement.
