Abstract

Laparoscopic sleeve gastrectomy (LSG) is a bariatric procedure that may allow similar results to Roux-en-Y gastric bypass (RYGB) and duodenal switch (DS), without problem of malabsorption. Several studies show that LSG is safe and effective in terms of weight loss and improvement of comorbidities, in the first post-operative years. For these reasons and for the fact that it does not imply any digestive anastomosis, LSG has become very popular among surgeons and has become so without a large knowledge base of long term outcomes.
Despite its wide diffusion, LSG’s long-term weight loss data are not uniform. This data could be in line with the fact that weight regain has been found after all bariatric operations. Now it is evident that weight regain following SG is significant. Yet the defining, reporting and understanding of this phenomenon remains largely neglected. Which surgical strategy to adopt in case of failure of the LSG? This question represents controversy in the literature and is still a matter of debate.
In case of insufficient weight loss or progressive weight regains. Recently, some authors suggested treating the inadequate weight loss, resulting from a large stomach or neo-fundus after LSG, with laparoscopic re-sleeve gastrectomy (LRSG). Laparoscopic re-sleeve gastrectomy is a feasible and effective intervention to correct weight regains after sleeve but long-term results of ReSG are awaited for further prospective, randomized clinical trials to improve efficiency.
