Abstract T his study was conducted to evaluate the effect of ball and socket attachment and Retention.sil material used with midline single implant supporting mandibular overdenture on its supporting structure. Fourteen male completely edentulous patients were selected to participate in this study. All patients participating in this study were rehabilitated by maxillary complete denture opposed by implant retained mandibular overdenture supported by single symphseal implant. Two types of attachments were used (Ball and socket attachment and Retention.Sil 400). A Single midline implant was positioned in the symphseal area of the mandible following the two stage surgical protocol. Patients were randomly assorted into two groups: group I: Patients received a single midline implant-supported overdenture and retained by ball and socket attachment. group II: Patients received a single midline implant-supported overdenture and retained by Retention. Sil 400 gr. Evaluation of the implant supporting structures was made at denture insertion, six, and twelve months after denture insertion by measuring peri-implant bone height and amount of posterior bone loss by using cone beam computed tomography (CBCT). The results of this study showed:- 1- Single implant-retained mandibular overdenture is a relatively simple treatment modality that simplifies surgical and laboratory procedure; it can be used for those edentulous patients who can’t afford multiple implant therapy. 2- Retention.Sil silicone material is most beneficial early periods of osseiontegration following implant loading in single implant retained overdenture. 3- Ball and socket attachment retaining a single midline implant showed lower peri-implant bone loss than those retained with Retention.
