Summary
The pattern of injury to the ankle joint depends on many factors including the age
of the patient, the quality of the bone, the position of the foot at the time of injury,
and
the direction, magnitude, and the rate of the loading forces.
The incidence of ankle fractures is increasing, and studies have
shown that fracture
incidence among the elderly has doubled over the last 40 years.
Among athletes, both profession
al and amateur, the incidence has also increased.
Due to its position and characteristics, the ankle is subject to numerous traumas, and its
fracture is the most frequent among the load
-
bearing joints.
Ankle injuries including distal fibular fractures
represent the most common injuries
of the lower extremity.
The pattern of injury to the ankle joint depends on many factors including the age
of
the patient, the quality of the bone, the position of the foot at the time of
injury, and the direction, magnitude, and the rate of the loading forces.
The vast majority of ankle fractures are sustained via rotational mechanism.
It is now
universally agreed that perfect reduction with rigid fixation is the cornerston
e in
preventing or at least delaying the onset of arthritic changes of the ankle joint after such
serious injury.
Various studies showed that the displacement of the talus faithfully followed that of
the lateral malleolus.
So f
ixation of the fibula ha
s since been proven to be the standard
for treatment of ankle fracture
-
dislocations.
The recommended operative treatment of oblique trans
-
syndesmoidal fibular
fractures includes fixation with lateral neutralization or posterolateral antiglide plates.
Posterior antiglide plating has been presented as alternative to lateral plating of distal
fibula fractures.
This prospective study included 20 patients who were admitted to Tanta University
Hospitals with closed distal fibula fracture type Weber
(B).
