Abstract Is it easy to full visualize the almost 360° of the femoral head and also the acetabulum?!; Yes we can do it by using the surgical hip dislocation. Orthopedic specialists who are not acquainted with this methodology might be worried about the possibility of avascular necrosis of the femoral head. However, precise learning of the vascular anatomy around the hip joint calms the surgeon. The deep branch of the MFCA is the primary source for the vascuarity to the head of the femur which is protected by the tendon of the obturator externus muscle. The artery is in safe zone as long as the trochanteric flip osteotomy is done anterior to the short rotators. The dislocation of the hip joint could be performed by other approaches. However, the Ganz approache has several advantages including: Abductor detachment done by a trochanteric flip osteotomy and a rigid fixation of this flip fragment restores immediate stability and allows for early mobilization of the patient The MFCA is protected by leaving all the external rotator muscles intact. Intraoperative monitoring of perfusion of the femoral head is possible.Surgical dislocation produces an anterior dislocation using low -grade controlled trauma. A little morbidity is associated with the technique and it permits the treatment of numerous conditions, which may not react well to different strategies. Safe surgical dislocation introduce a new knowledge about the pathology of some hip disorders and also the ability of preserving the hip with techniques such as cartilage transplantation. Hip arthroscopy is picking up prominence in the management of hip disease. Later on and with more experience, progression in surgical abilities and the developments in instrumentation, arthroscopy might overcome the surgical hip dislocation approach. A further studies is needed to get well knowledge about the Indications, advantages and disadvantages of the surgical hip dislocation in comparison with hip arthroscopy. |
