Abstract

Introduction: Hepatocellular carcinoma (HCC) is the most common primary liver cancer, the fifth most common cancer and the third most common global cause of cancer- related deaths. Currently, there are three potentially curative therapeutic options for HCC, liver resection, orthotopic liver transplantation (OLT), and local ablative therapies.
Aim of the Work: This work аims to discuss thе rolе of livеr transplаntation in mаnagement of HCC аccording to Milаn’s аnd thе expаnded criteriа аnd compаre thе usе of thе currеnt criteriа (both the Milаn and UCSF) for thе sеlection of HCC pаtients for possiblе OLT.
Methodology: Based on current data, general exclusion of patients with HCC beyond MC from LT is no longer justified. In fact, the implementation of the MC in 1996 had a tremendous impact on establishing HCC as major indication for LT. Patients with HCC meeting the MC may achieve 5-year recurrence-free survival rates of about 70%.
Conclusion: Patients with HCC and a cirrhotic liver but without extrahepatic disease should be candidates for liver transplantation whenever possible. Although Milan criteria were used as the standard criteria for selection of patients with HCC eligible for LDLT, it seems to be too restrictive. Patients with single tumor less than 7cm or have multiple lesions with total tumor burden to be less than 8cm have no significant difference in survival when compared with patients who were selected for LDLT according to Milan criteria.
