Abstract

Standard or conventional HD prescription is far from being optimal in replacing the function of normal kidneys. Its unphysiologic clearance pattern and inability to remove all types and sizes of uremic toxins results in inter and intra-dialysis complications.
At present, online hemodiafiltration (HDF) offers an ideal renal replacement therapy (RRT) option for end stage renal disease (ESRD) patients. By combining diffusive and convective transfer in the same exchange module, HDF provides the highest clearances of both small and large solutes .The online production of sterile and non-pyrogenic fluid by ultrafiltration has several advantages; technical aspects of HDF are simplified,the overall cost of HDF is reduced, bicarbonate-buffered infusate facilitates the correction of acidosis and Finally, online HDF provides a multipurpose treatment option.
Our study is a comparative study aimed at assessing Β2-microglobulin and phosphorus in patients with ESRD undergoing haemodiafiltration compared to those undergoing conventional haemodialysis and to introduce a new technique of dialysis in dialysis unit of Assiut University Hospital.
Our study was carried out from February 2012 till December 2013 by selecting 47 patients with chronic renal failure on regular hemodialysis for at least six months,non diabetic ,aged 18-60 years, and classified into two groups: group(A), 23 patients (aged 38.24 ± 11.02; 15 males & 8 females) shifted from conventional HD to post-dilution HDF and group(B), 24 patients (aged 41.76 ± 9.25; 14 males & 10 females) continue their conventional HD rigmen(control group) and the two groups were on 3 sessions/week 4 hours each, rigmen and subjected to full history and clinical examination ,routine investigations in addition to measurement of serum Β2-Microglobulin and Phosphorus level at the beginning of the study and after six months(the duration of the study), and the procedures were carried out at the dialysis unit of Assiut University Hospital.
There was a significant reduction of both β2-Microglobulin and phosphorus after shifting from conventional HD to post-dilution HDF, in the group A compared to the group B ( the control group), also there was a significant reduction of both substances within the same group of patients ( group A) ,before& after six months (the duration of the study).
We recommend to replace the conventional HD with OL-HDF in all patients with ESRD especially those with high level of B2-MG and phosphorus ( e.g. with severe itching or B2-MG amyloidosis &or carpal tunnel syndrome), if financial resources are available and/or affordable.
