Abstract

We recommend the consideration of endoscopy in managing the facial fractures at least as a helping tool unless as a different modality of approaching the fracture repair. It may also be of great value to use the endoscope in treating facial fractures other than included in our study like the subcondylar mandibular fractures. Development of instrumentations specific for Maxillofacial Surgery needs to be encouraged. We agree with Strong 2008 in that the screw must be securely attached to the screwdriver to avoid dislodging the screw as it passes through the soft tissue. Further investigations; including long-term follow-up, and a larger series of patients, are needed with further researches. The surgeon must use meticulous surgical technique and avoid excessive traction or manipulation of critical structures as the orbit and the infraorbital nerve. Regarding the treatment of the facial fractures, we advise the early management of the fractures as possible to avoid the undesirable fibrous tissue formation which prone to the difficult dissection and reduction of the fractured bone. This also should be weighed against the relief of the edema and the associated swelling of the surrounding soft tissue. Upon using the endoscope in management of facial bone fractures, surgeons should be familiar with the endoscopic usage as observed in ORL surgeons; on the other hand, they should be also familiar with implant fixation including the handling of the plates and screws as observed in plastic surgeons. As a result, we encourage the team working and cooperation between ORL and plastic surgeons to obtain the best results and to interchange the experiences & surgical skills.
