Abstract

Background: Endometriosis affects 10% of the females in the reproductive age. The common presenting symptoms are infertility and chronic pelvic pain. Laparoscopy is the gold standard for the diagnosis and staging of the disease but it may have some complications. Ultrasonography could be an alternative tool for diagnosis. It is safe, cheap and simple procedure with no complications.
Setting: Mansoura university hospital.
Patients and methods: 158 patients in childbearing period complaining of infertility included in our study. They were booked for laparoscopy.Transvaginal sonography was done for all patients. When endometrioma, Douglas pouch obliteration, fixed ovary, kissing ovaries or thickened uterosacral ligament was detected, it was scored. A comparison between sonographic findings and laparoscopic findings was done.
Results: Transvaginal sonography showed a high diagnostic accuracy of endometrioma, Douglas pouch obliteration, fixed ovary, kissing ovaries. The sensitivity, specificity, PPV, NPV and accuracy of the transvaginal sonography for diagnosing thickened uterosacral ligament was 64%,88.8%, 72%, 84% and 81% respectively. There was a very good level of agreement between transvaginal sonography and laparoscopy in staging pelvic endometriosis as an advanced stage
(moderate and severe) (k index 0.9). Also there was a good level of agreement between transvaginal sonography and laparoscopy in staging pelvic endometriosis as an early stage
( minimal and moderate) ( k index 0.6)
Conclusion: Transvaginal sonography is an accurate method for diagnosing late stages pelvic endometriosis. It could diagnose early stages with a good accuracy.
