Abstract • The human genetics play a major role in inter-individual variability in the course of human infections . • For over 50 years many genetic studies have been used to identify loci for many major causes of human infectious mortality. • Genetic controlling of infections occurs through interaction between the infectious pathogen and different immunological pathways. • Molecular genetic testing can supply genetic information to predict IDs risk in individuals. • Genetic counseling plays a critical role in the appropriate use of these tests. • Pre symptomatic and predictive genetic testing or carrier genetic testing of minors is the subject of multiple international guidelines and position papers. • It is important to stress that such predictive testing can be recommended if the results are of direct benefit to the minor, through either access to specific treatment or to preventative therapy . • Pre-implantation genetic diagnosis of IDs susceptibility has many benefits for the couple, and is likely to become more successful in the future. • Prenatal diagnosis of IDs susceptibility enables a couple to identify a susceptible fetus and then make decisions about the outcome of that pregnancy. If the couple chooses to continue the pregnancy, early treatment of a susceptible child can occur. • Universal newborn screening for SCID is waited with great interest . • Recently there has been interest in community screening for SCID. • Screening of at-risk relatives will be beneficial in early detection of IDs in certain families. • We need to create the next generation of clinician -scientists who are capable of translating discoveries of bench research to solve clinical problems in the ward. • Personalized medicine will soon be a reality in the near future in the field of diagnosis, prophylaxis, and therapy. |
