Abstract Goldblatt defined a causal relationship between renovascular disease and hypertension through his innovative work published in 1934. Renal artery stenoses are due to atherosclerosis or to fibromuscular dysplasia. Atherosclerosis is the most common cause of renal vascular disease, accounting for approximately two thirds of cases. It affects older age group, more common in men, smokers and often associated with renal impairment. Fibromuscular dysplasia accounts for approximately one third of cases. It affects younger patients with approximately three fourths of cases occur in women. It rarely causes renal impairment. Renovascular disease continues to challenge the clinician . Specialists have known for a long time that renal artery stenosis (RAS) is the major cause of secondry hypertension and that it may account for 1-5% of cases. RAS also is recognized increasingly as an important cause of chronic renal insufficiency and end-stage renal disease. Although renovascular interventional procedures are now widely available and of benefit to some people with renovascular disease, a large proportion still progress to dialysis. |
