Abstract

The primary goal of bariatric surgery is to induce sufficient weight loss to ameliorate obesity-related comorbidities such as diabetes, heart disease, hypertension and obstructive sleep apnea .
Bariatric surgery has become an attractive long-term solution for many patients for managment of obesity.
Bariatric surgery leads to reduced caloric intake by modifying the anatomy of the gastrointestinal tract. Specific operations are classified as either restrictive or malabsorbtive. Restrictive procedures include stapled gastroplasty and adjustable gastric banding. Malabsorptive procedures include biliopancreatic diversion and duodenal switch.
Although these procedures can be successful, they are technically difficult and can be associated with alot of complications.
In the mean time it is imperative that the gastroenterologist and surgeon work closely together cooperatively in order to recognize and treat these complications early in order to optimize outcomes and decrease morbidity and mortality in this challenging group of patients.
Use of endoscopic interventions for the treatment of obesity is in its infancy, but will eventually provide valuable approaches to presurgical weight loss, postsurgical revision of previous weight loss procedures, and primary surgeries. Such interventions may offer economical, minimally invasive outpatient options for growing numbers of weight loss surgery patients .
Transoral gastroplasty is part of a new movement in surgery that uses natural orifices (like the mouth) to perform surgery instead of cuts and slits in the skin.
