Abstract

Neopterin is produced by these macrophages and though to represent a marker of immunity activation and macrophage activity. Also it enhances the inflammatory activity within vulnerable plaques and stimulates gene transcription for inducible nitric oxide which releases unstable cytotoxic NO free radical .
Neopterin has been shown to be elevated in the serum of patients with unstable angina and acute myocardial infarction as compared with control subjects and patients with stable angina pectoris.
This study aimed to evaluate the potential role of neopterin in the modulation of atherosclerosis process, namely in the pathogenesis of ACS and as a marker of instability. The study aimed to identify patients at high risk and will benefit from more aggressive therapeutic approach.
In this study 40 subjects presented with NSTE ACS were admitted and matched for age and sex. All patients were subjected to complete medical history, full clinical examination, laboratory investigations including (Sr. cholesterol, LDL, HDL, TG, sr.creatinine, Blood glucose, and CBC, ESR, CK MB, SGOT, LDH and serum neopterin), ECG, TTE and coronary angiography.
In the present study there was a highly significant correlation between neopterin levels and adverse outcome. Also significant correlation between neopterin levels and complex lesions while it was significantly lower in vessel score.
These data suggest that higher levels of neopterin concentrations are associated with clinical instability and favor the development of atheromatous lesions with signs of instability.
Undoubtly, the association between inflammation, coronary plaque instability and clinical presentation is extremely complex. However, these findings highlight on interesting relationship between clinical presentation wit ACS and elevated levels of neopterin.
